Sports Physicals
Complete Student-Athlete Package — $199
Your visit includes:
Comprehensive sports physical
A preparticipation examination performed by a board-certified family physician, including:
- Medical and family-history review
- Cardiovascular risk screening
- Blood pressure and vital signs
- Heart and lung examination
- Musculoskeletal examination
- Evaluation for safe athletic participation
- Completion of the FHSAA EL2 physical form
12-lead EKG screening
A medical-grade, resting 12-lead EKG performed in our office.
Cardiologist interpretation
Your EKG is submitted for review by a board-certified cardiologist. Because the cardiology report may not be immediate, final paperwork could require same-day or next-business-day completion.
FHSAA paperwork
We complete the applicable EL1 EKG screening form and EL2 participation physical form. The EL1 becomes required beginning July 1, 2026, for incoming ninth graders and qualifying new athletes in grades 10–12.
Who needs the new EKG?
The one-time screening generally applies to:
- Incoming ninth-grade students participating in FHSAA athletics
- Students in grades 10–12 participating in FHSAA athletics for the first time
- Students who have not already met the EKG requirement through an eligible prior screening
Students with an abnormal EKG cannot be cleared for participation until the required follow-up and written medical clearance are completed.
What should you bring?
Please bring:
- Completed parent and athlete sections of the current FHSAA forms
- Any glasses, braces, inhalers, or other athletic equipment used by the student
- A list of medications and medical conditions
- Relevant cardiology or orthopedic records
- Information concerning any family history of sudden cardiac death, unexplained fainting, cardiomyopathy, or serious arrhythmia
Minors must be accompanied by a parent or legal guardian unless arrangements have been approved in advance.
What happens if something is abnormal?
An abnormal result does not necessarily mean that a student has heart disease. Some findings may require additional evaluation before athletic clearance can be issued.
When further evaluation is needed, we will explain the result and provide the appropriate referral recommendations. Additional testing, specialist visits, and completion of supplemental clearance forms are not included in the $199 package.
